For a survey to be available for sending from an item, a suitable template needs to be created first.
Survey Templates can be created either from Categories and Crowds > Crowd ellipses (...) > Survey Templates or by going to the Crowd Profile page for your Crowd and choosing the Survey Templates option the ellipses (...) menu.
You'll then see a page similar to the example below.
From here, you will be required to give your template a title and a short description. You can also select an image to go along with your template. The Survey Theme Colour allows you to customise the colour scheme of the survey to your organisation's branding. Either use the colour wheel, RGB or Hex codes to get the exact colour you want.
You will also be required to enter an introduction message. This can help to give a little more context and clarity on what the aim of the survey is.
To begin adding questions, you will first need to add a section and give it a title.You can then add your question, for which you will have 6 choices; Choice (Single), Choice (Matrix), Choice (Multi), List of Values, Long text or Short Text responses
Choice (Single) offers radio button response options to survey respondents, meaning that only 1 option can be selected. This option also allows scores to be associated with each option.
Choice (Matrix) offers radio button response options within a matrix of question sets for a question, meaning that only 1 option can be selected for each question set. Scores can be associated with each option.
Choice (Multi) offers check box response options, meaning that more than 1 option can be selected by survey respondents.
List of values presents single option responses (similar to Choice Single) but in a drop down list of values, and does not allow scores to be associated with each option.
Long Text (Multi-line) offers a text area for question responses.
Short Text (Single-Line) offers a single line text field for question responses.
The example below shows a Choice (Matrix) question, with 4 scored options, and 3 question sets within the Matrix.
When you add your question, you can also use the More Info field to specify what this means if necessary. Then, depending on your question type, you will add the answer options and add a score if appropriate. Select whether or not this question is required of users answering, and also provide the option for users to add a comment (based on their answer.
Use the Trash symbol to delete a section, question or a question set, the Copy option to add a new identical question to the template, and the up and down arrows to reorder the sections, questions or question sets to appear in the order that is best and most coherent.
Your survey template will save as you go, so just click the back button when the template is complete and it will be ready for adding to an idea!
